O R D E R

On November 2, 2001, an Initial Decision was issued in which it was concluded that all mail addressed to Cedar Avenue Physical Therapy, Inc., 5304 Cedar Avenue, Philadelphia, PA 19143-1918 should be delivered as directed by Saleem Shakur and mail addressed to Spruce Street Physical Therapy Center, Inc., 6100 Spruce Street, Philadelphia, PA 19139-3740 should be delivered as directed by Cherie Shakur.

Since no appeal has been filed and the Judicial Officer has determined he will not exercise his discretion to review the Initial Decision on his own motion, the Initial Decision has become the final agency decision as provided in 39 C.F.R. §965.12.
