SF-1152
Unpaid Compensation of Deceased Civilian Employee
Payment of a deceased employee's last pay check to include the payment of earned annual leave and any other miscellaneous monies payable to beneficiaries.
The completed original SF-1152 must be submitted to the employee's servicing personnel office for immediate filing in the Official Personnel File (OPF). The servicing personnel office will certify the form and mail a copy to the employee's mailing address of record.

SF-2808
Civil Service Retirement System
Lump sum payment of employee's retirement contributions if no entitlements exist for a surviving spouse and/or dependent unmarried children under the age of 18, or under the age of 22 if a full time student, or unmarried and incapable of self support (regardless of age, provided the disability was established before age 18).
The completed original SF-2808 must be submitted to the U.S. Office of Personnel Management address shown on the form for review, certification, and filing in the employee's retirement account folder. OPM will mail a copy of the certified form to the employee's mailing address of record.

SF-2823
Federal Employees Group Life Insurance (FEGLI) Program
Payment of the value of an employee's life insurance benefits based on the coverage in effect at death. If you assigned ownership of your life insurance to another person, or if you did not assign your life insurance but there is a valid court order that governs payments of your benefits, then those have priority over any FEGLI designation of beneficiary form you complete, as explained on the form.
The completed original SF-2823 must be submitted to the employee's servicing personnel office for immediate filing in the Official Personnel File (OPF). The servicing personnel office will certify the form and mail a copy to the employee's mailing address of record.

SF-3102
Federal Employees Retirement System

TSP-3
Thrift Savings Plan
Payment of an employee's TSP account balance as of the date of death.
The completed original TSP-3 must be mailed to the Thrift Savings Plan address shown on the form for review, certification, and filing in the employee's TSP account file. Verification of any valid TSP beneficiary form on file will be reflected on the employee's future TSP account statement.
