What you will need:


Proof of U.S. citizenship (provide one of the following):

  • Previous U.S. Passport
  • Certified U.S. birth certificate (with raised seal)
  • Consular Report of Birth Abroad or Certification of Birth
  • Certificate of Naturalization or Citizenship

Proof of Identity (provide one of the following):

  • Current valid Driver’s License
  • Government or Military ID
  • Certificate of Naturalization or Citizenship
  • For minors under 14 years of age, each child must appear in person and both parents or legal guardians must appear with them or provide notarized statement of consent and present evidence of identity

We will have cameras available to take required passport photos - $15 fee**
